The Practice provides internal medicine, weight management, and related health services, which
may include evaluation, diagnosis, treatment, and medication management, including telehealth
services where appropriate.

Dr. Makartian is a licensed physician in the State of New Jersey. As a physician, Dr. Makartian may prescribe medications when clinically appropriate and mutually agreed upon as part of your treatment plan. If at any time you have questions regarding your treatment plan, please feel free to discuss them with your provider.
TELEHEALTH SERVICES
I understand that some services may be provided via telehealth (video, phone, or electronic communication). While telehealth improves access to care, it may have limitations, including the inability to perform certain physical examinations and potential technical or security risks. I consent to receive care via telehealth when deemed appropriate.
CONFIDENTIALITY AND PRIVACY
Your privacy is important to us, and your information will be handled in accordance with applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA). There are some exceptions where information may be disclosed without your consent:
- If the Practice believes there is a serious risk of harm to you or others.
- If there is suspected abuse or neglect of a child, elderly individual, or vulnerable adult, as required by law.
- If required by court order, subpoena, or other legal process.
- If another healthcare provider needs information to provide care in the provider’s absence.
- If you provide written authorization to share information for coordination of care.
